Hi Sudipta..Can you plz check the ORACLE_SID, ORACLE_HOME, TNSPING for the SIDADM? Also the file level permissions for SIDadm/ORAsid(unlikely as the DB is running)
Check the tnsnames.ora for RP2 if everything is correct there. Again check the running listener - goto status and check if it matches correct RP2 values/references?

-As of now by going through the log it seems that the R3trans is able to fetch the basic info of the oracle(upgraded version-patches) which is similar to getting into sqlplus without any logon..but when R3trans is trying to logon it's not able to determine the instance so above mentioned one/some is surely to be blamed for.
